Out of Hours Works Request – Capital Projects Only

When to use this form?

Use this form to request approval for Capital Projects works to be undertaken outside standard operating hours.

Out of hours request must be submitted at least 2 days prior to the proposed works to allow sufficient time for review and approval.

Submission of this form is a request only and does not constitute approval. Out of hours works must not proceed until formal approval has been provided by the Director Capital Projects (or delegate)

All works must be completed in accordance with all relevant WHS requirements, Curtin University requirements and approved controls.

Enter the relevant CP number, or N/A if not applicable.
Include reason why works cannot be completed during ‘normal operating hours’
Confirm worker fatigue will be monitored and what controls are in place to mitigate fatigue
Contact details of supervisors who will be present on site during the works
Identify any additional hazards specific to weekend works and what controls have been implemented
Permits associated with works(Required)

Confirm that any necessary stakeholder consultation has been completed
i.e Fatigue or Risk Management Plan
